The Regional Geriatric Program of Eastern Ontario, in partnership with the Champlain Local Health Integration Network (LHIN) and a broad group of community partners have led the development of a Champlain Falls Prevention Strategy.
Patient & Families
This section is for members of the public who want to know more about Fall prevention and how to reduce the risk of falls. The Champlain Stay on Your Feet Fall Prevention initiative provides helpful tools and tips including:
- Learn how You can Prevent Falls.
- Complete/print a screen to assess your risk of falling take to discuss with your health care provider.
- Find exercise programs in your area to improve your balance and strength.
- Learn how to check your home for potential hazards.
Health Professionals
For Health Professionals who want to increase their knowledge of fall prevention to screen, assess and intervene for older adults. In this section you will find tools, tips and resources to help determine and treat underlying causes of falls including:
- How/where to refer your patient to Specialized Geriatric Services
- Screening, assessment and intervention resources to help you asses your patients multifactorial fall risk.
- Resources to provide your patients e.g. “You Can Prevent Falls”
- Professional development education modules for every level of health professional
The Regional Geriatric Program of Eastern Ontario (RGPEO), in partnership with the Champlain Local Health Integration Network (LHIN) and a broad group of community partners have led the development of a consistent fall prevention strategy for older adults for this LHIN. The SOYF® Program was adapted with permission as a framework and builds on the emphasis by the Ontario Ministry of Health and Longterm Care on fall reduction and reducing the risk for falls.
The SOYF® program guided the Champlain Fall Prevention Steering Committee to develop a fall prevention strategy and to look at gaps, challenges and enhancers from community care, all the way to longterm care.
