The Community Geriatric Coordinator is a community-based social worker that provides support to older adults and their caregivers.


Reasons for referral include:

  • Advocacy and Referral/Navigation
  • Family and Social Supports/Caregiver Stress
  • Future Planning
  • Financial Insecurity
  • Self Neglect
  • Mood/Grief
  • Power of Attorney
  • Adjusting to New Diagnosis
  • Advanced Care Planning
  • Suspected/Known Abuse

Referral Criteria

  • Older adults must live in the community in the geographic areas of Renfrew, Barry’s Bay, hwy 60 corridor, or Cobden.

Referral Process

Anyone can refer an older adult to our program with their agreement. Completed CGC Referral Form should be faxed to 613-432-7510.

The referred older adult will be contacted to arrange an initial appointment in their home, in the hospital clinic, or by phone, depending on individual needs. Older adults are discharged from service once individual goals are achieved or supports are no longer required.
