Central Intake for Specialized Geriatric Services (Geriatric Medicine)
Specialized Geriatric Services (SGS) provide older adults (65 years of age and older) with specialized care and services that are often necessary to prevent, delay or restore loss of function. Outpatient SGS include but are not limited to Geriatric Day Hospitals, in-home Geriatric Assessments, GeriMedRisk, Memory Programs, and Geriatric Clinics. Each SGS has varying patient criteria and accept referrals from different sources.
Central Intake for SGS will receive, triage and direct referrals to the most appropriate SGS program.
Central Intake for SGS (Geriatric Medicine) is designed to be a simplified streamlined intake process with one referral form and one contact number. All referrals sent to Central Intake for SGS will be triaged by a health care professional. A decision to refer to a Specialized Geriatric Service is made based on the review of the referral form, medical information and a discussion with the older adult/caregiver.
Older adults are referred by their Primary Care Practitioners, other healthcare professionals, family, caregivers or themselves. Referrals are accepted for people who live in their own home (house, apartment, condominium, retirement home). Referrals cannot be accepted from long-term care facilities and out of province.

A Geriatric Assessor completes a comprehensive geriatric assessment on adults who are 65 years or older.
Older adults are referred by their Primary Care Provider, other healthcare professionals, family, caregivers or themselves. Referrals are accepted for people living in the community. Referrals cannot be accepted from long-term care facilities or out of province.
Our Outreach Teams are committed to working with the Primary Care Provider and community services agencies.
Geriatric Clinics & Day Hospitals
Geriatric Clinics and Day Hospitals are out-patient programs that provide multidisciplinary assessment and/or treatment for older adults experiencing a change in function, memory, mood, mobility or have complex medical issues. Treatment, counselling and education are available to older adults and their caregivers to optimize health and independence.
Before an appointment is made for a Geriatric Clinic or Day Hospital, many older adults have already been seen in their own home by the Geriatric Outreach Assessor . Referrals may also be made through the Emergency Department Geriatric Emergency Management (GEM) Program by family physicians or from in-patient units. A physician referral is required.

Cornwall Community Hospital Geriatric Day Hospital
The Geriatric Day Hospital is a specialized geriatric service funded by the Regional Geriatric Program of Eastern Ontario providing comprehensive multidisciplinary geriatric assessments for older adults, 65 years old and over, who are experiencing a change in function, mobility, memory, mood, or have complex medical issues.
Referrals are accepted from family physicians, health care professionals and community agencies (ER physician may request a GEM assessment). Urgent assessments are available

Winchester Memorial Hospital Geriatric Clinic
Dr. Genevieve Casey is a Geriatrician who provides a monthly Geriatric Clinic for local patients. A Geriatric Assessment must be completed by Home and Community Support Services Eastern Counties prior to being seen at this clinic. Older adults are referred by their Primary Care Provider, other healthcare professionals, family, caregivers or themselves. Referrals are accepted for people living in the community. Referrals cannot be accepted from long-term care facilities or out of province.

Mobile Geriatric Day Hospital
The Renfrew County Mobile Geriatric Day Hospital is an out-patient program that focuses on health promotion and offers practical strategies aimed at preventing debilitating illness or injury. Services from counselling, memory and safety assessments to individualized exercise programs that minimize risk for falls all aim to promote our seniors’ independence. Our professional team is here to guide patients and their families throughout their health journey every step of the way.

Almonte Geriatric Day Hospital
The Day Hospital is a therapeutic goal-oriented Rehabilitation Program for adults requiring inter-disciplinary assessment, treatment support and teaching to adapt and cope with health changes related to aging, acute or chronic illness. Services provided include: Nursing, Recreation Therapy, Physiotherapy, and Occupational Therapy.
The Day Hospital team collaborates with other community health services such as the LHIN Home & Community Care, Lanark County Mental Health, Health Links and Home Support. Attendance is two days per week for a prescribed duration, followed by discharge to community-based wellness programs. The goal of the Day Hospital is to increase each client’s quality of life and independence so that they can continue to live in the community.

Renfrew County Community Geriatric Coordinator
The Community Geriatric Coordinator is a community-based social worker that provides support to older adults and their caregivers. They are responsible for building capacity and improving transitions and access to services with a focus on persons and caregivers living with dementia. This will involve collaborating with partners at the operational and patient level to bring forward system issues for discussion and action. The Community Geriatric Coordinator will ensure that the delivery of services is provided to the client population by supporting throughput of complex patients.
Referral criteria include older adults must live in the community in the geographic areas of Renfrew, Barry’s Bay, hwy 60 corridor, or Cobden.
Anyone can refer an older adult to our program with their agreement. Completed CGC Referral Form should be faxed to 613-432-7510.
